  • Uncomplicated UTI:
    Acute cystitis in afebrile nonpregnant premenopausal women with no diabetes and no urologic abnormalities
  • Acute Pyelonephritis:
    Acute kidney infection in women otherwise meeting the definition of uncomplicated UTI above
  • Complicated UTI:
    All other UTIs

  • Uncomplicated UTI:
    Infection confined to the bladder in afebrile women or men
  • Complicated UTI:
    Infection beyond the bladder in women and men
    • Pyelonephritis
    • Febrile or bacteremic UTI
    • Cather-associated (CAUTI)
    • Prostatitis (not covered by these guidelines)

There are ~3 million cases of cUTI in the US6

The heterogeneous nature of cUTI can make clinical management challenging7,8

Patients may present with atypical symptoms, comorbidities, and complications, which should be considered on an individual basis7,8

Patients with cUTI and acute pyelonephritis may also present with7-9:

  • Bacteremia
  • Sepsis or septic shock
  • Multiple organ system dysfunction
  • Acute renal failure

Complications are more likely in patients with the following characteristics8,9:

  • Urinary tract obstruction
  • Instrumentation (catheters)
  • Immunosuppression
  • Older patients
  • Diabetes mellitus

Complications can lead to rapid and severe disease progression

e.g., obstructive pyelonephritis can progress to urosepsis, septic shock, and death without timely surgical intervention7

Antimicrobial Resistance in cUTI Limits Oral Treatment Options

AMR and MDR uropathogens in cUTI can lead to treatment failure, increased hospitalization rates, and progression to more severe stages of disease, such as sepsis 10-13

Rising AMR against current first-line cUTI therapies have led to increasingly limited effective therapeutic options, particularly oral options, for some patients 9-11
